All Seasons Hafla Application

This form should be filled out at least 6 WEEKS before the desired show. Accepting applications for February and March '09 now. The first 8 items are required, and there must be at least 100 characters in the Bio section. feel free to make it longer! After filling out this form, you will receive a confirmation email with the information you have submitted.

First 6 applications received for a certain month will be booked.

Please bring a CD with only that day's music on it, labeled with your name.

Students: 3-6 minutes; Headline: 15-18 minutes, Other dance professionals, in between!

Include current teachers/inspirations/choreographer, if applicable. This can be modified later, but give me something I can post before your performance.

This is so we don't have 2 of the same song. If you don't know yet, check with me when you pick it.
